There are four forms that need to be submitted for signing up for the entire school year:

• Emergency Release
• Program Sign Up
• Cancellation Agreement
• Behavior Agreement

In addition we have sign up forms for each individual session that indicates which activities a student wishes to take, plus a parent signature so we know that the parent is aware of the student being involved with AfterSchool for that session.

All five of these forms must be handed in before a student can start the program.

Forms can be submitted four ways:

• Mail to AfterSchool Program at 43 Main Street, Ludlow, VT 05149
• Faxed with Attention to AfterSchool Program at 802.228.7233
• Dropped off at the High School Main Office
• Given to your child's teacher
